This concussion training presentation is for reference only and does NOT count for completion of Concussion Education. You must still attend a face to face training or take the online training if applicable. Info can be found under concussion information under the links section.

Fall 2019 Parent Meeting

VHSL Physical Form for the 2019-2020 School Year

All physical examinations must take place after May 1, 2019 for a student-athlete to participate in sports at Patriot for the 2019-2020 school year. The examination results and physician's signature must be on this form and submitted to the activities office prior to tryouts.

Concussion Face to Face Training Dates for 2019-2020

The face to face concussion training schedule has been posted under the files section of our website. All rising 9th graders and new students to PWCS must attend a face to face training with a parent or guardian prior to tryouts for the 2019-2020 school year.  All upperclassmen who have attended a PWCS high school face to face training in the past, can take the online training which should be available around July 1st.  The link will be posted under the links section of our website.
