Physical Form for the 2022-2023 School Year -
All physical examinations must take place after May 1, 2024 for a student-athlete to participate in sports at Patriot for the 2024-2025 school year. The examination results, all information, and physician's signature must be on this form and submitted to the activities office prior to tryouts.
